ROY, Wash. -- A Washington feed mill where a silo collapsed while two men were doing maintenance on it was cited earlier this year for six serious safety violations.
Authorities say one of the men is presumed dead after Monday s collapse. The other worker and two people in a nearby office escaped.
Records released Tuesday by the state Department of Labor and Industries show the mill s owner, Wilcox Family Farms, was assessed a $10,000 penalty after a June review.
Inspectors cited the company for having inadequate rescue services around a corn tank, and for failing to fully inform employees of the hazardous conditions.
Wilcox Family Farms supplies eggs throughout the Northwest. The company hasn t responded to a call seeking comment.
